The awards program recognizes the most successful and innovative case management programs working to improve health care across the care continuum and sets the standard for programs that deliver sustained success.

ComForcare Senior Services is one of four finalists in the nation to be nominated for an award in the 2012 “Managed Care: Transition of Care Programs” category, which recognizes programs that have made inroads to improving the transitions of care.
